Keep Your Medicines List
You can help to promote medication safety through engaging with patients to keep a medicines list and bring it to appointments and admissions.
Empowered patients who know and understand their medications can greatly reduce medication-related harm. People who use a medicines list are more likely to be able to recall their medicines accurately at admission than those who do not, reducing their risk of medication error at transitions.
