The HSE is reviewing the transition of care young people receive when moving from paediatric to adult health services. They are seeking pharmacists support in highlighting their ‘Your Voice Matters’ questionnaire!
The questionnaire is aimed at young people aged 18-26 years, who are currently in the process of or have already transitioned from a paediatric to an adult health care service, and their family members, carers or friends. The HSE have asked for your support in displaying this poster and raising awareness with your patients.
